Conditions Prior to Occurrences.
Unit 2 shutdown..
Description of Occurrencet During investigation.of a suspected tube leak in the.. "D" RHR heat exchanger thage was apparently some v.igration of reactor water into the High Pressure Service Water ;
System side of the heat exchanger. As soon as a Icak was positively identified -
the heat exchanger was removed from service and isolated.
Designation of Apparent. Cause-of Occurrence:
Tube leak in
'D' RER beat exchanger.
Analysis of occurrence:
There was mininal safety significance associated with this tube failure. Initial water analysis indicates.a low value of microcuries released.
Corrective Action:
Beat evenanger undergoing tube xepair.
Failure Date:
None previous - liquid release.
