. Failure of Pri=ary Containment Isolation-Valve.AO-3969A to.close.
Conditions-Prio'r to Occurrence:
Plant operating at 55% Power.
Description of Occurrence:
During the performance of the PCIS normally oper valves surveillance
-test. the instrument N header _to dryvell AO '3969A failed.to close 2
when actuated from the remote control switch.x Air.was manually' vented
^
and' the valve closed.
It was then.~ operated satisfactorily six times
- from the remote control switch.
Designation of Apparent Cause of Occurrence:
Under. investigation.
Analysis of Occurrence:
There are two check valves in series.downstreas of AO-3969A in the instrument'3 header to the dryvell.
These check valves would prevent 2
l 1eakage from the drywell to the reactor building, therefore, the failure has minimal safety implication. A Corrective Action:
The solenoid valve will be disassembled, inspected, maintained, and retested.
