4 Description of Cccurrence:
During the perfor=ance of a surveillance test, AO-5235 failed to close when given an isolation signal.

Designation of Apparent'Cause of Occurrence:

The solenoid valve for operation of AQ-5235 was apparently dirty becat subsequent cycling of the valve caused it to operate correctly.
The cause of the failure is presently under investigation.
