Abnormal Occurrence No. A0-72-08 l
Dear Sir:
I As defined by Technical Specifications for the Vernont. Yankee Nuclear Power Station, Section 6.7.B.d, we are reporting the following abnormal occurrence as A0-72-08.
j On October 20 during conduct of the Automatic Depressurization System
'B' Logic Test, the Relief Valve Initiation Time Delay Relay (No. 2E-K5B) failed to energize in < 120 seconds as required by a specification in Table 3.2.1 of Technien1 Specifications. At this time the reactor was operating i
at approximately 50% power.
l The relay was immediately reset and was functionally tested and verified i
to be operable at a time delay of 117 seconds. This remedial action was completed in a time span that precluded the necessity of further immediate action.
Investigation of the calibration records revealed that the time j
delay of the relay in question was within specifications at the time of the j
last surveillance test in April, 1972. There was no previous history of this I
time delay relay being out of specification.

VERMONT YANKEE NUCLEAR POWER CORPORATION Directorate of Licensing October 26, 1972 Page 2 The Plant Operations Review Committee concluded that although this was probably an isolated incident, and although the results of the calibration history review do not support a change in testing frequency, a single check of the timer would be conducted three months after the incident to confirm timer reliability.
Very truly yours, VERMONT YANKEE NUCLEAR POWER CORPORATION e\\t3, B.W. Riley Assistant Plant Superintendent DUE/rph i
