On June 8,1995, Channel 13 (S/G 1B Manway) was declared inoperable.
Trending of bias voltage and background RMS was evaluated to conclude that the channel was experiencing periodic failures. Problem Investigation i
Process (PIP) 1-C95-0883 has been initiated for tracking purposes; work request 95025569 (WO 95045842-01) has been initiated to perform corrective maintenance on this instrumentation. This work will be performed during 1EOC9 outage.
Redundant monitoring is afforded by Channels 12 (S/G 1B l
Secondary) and 14 (S/G 1B Hot Leg).
i Since this channel has been inoperable for 30 days; it is reportable pursuant to Technical Specification 3.3.3.9.
