Identification of Occurrence:

Failure of the RCIC inboard isolation valve Mo-3113-15 to open.
f Conditions Prior to Occurrence:
Unit 3 at approximately 555 power.
Description of Occurrence:
During the performance of a curveillance test. MO-3-1Y-1.5 was intentionn elosed by an isolation scram.
Following the reset of the isolation.
the valve could not be reopened.
Designation of Apparent Cause of Occurrence.
Under investigstion.

Safety i=plications are minimal because RCIC is backed up by the HPCI r
l syntes, and becau:c the systen was returned to service t h e s ame d ay.
Corrective Action:
The additional surveillance required by a inoperable RCIC system was initiated.
Following a plant scras not associated witn this abnormal l
l, occurrence, the RCIC valve was reopened.
