-s March 27, 1975 Page62 Designation of Apparent Cause of Occurrence:

Sticking switch contacts repaired by cleaning.

Analysis of Occurrence:

The companion switches in this logic were tested successfully; therefore, the safety implications of this f ailure are minimal because this switch is connected in a one-out-of-two twice logic and would not have prevented an isolation.

CoFrective Action:

The switch was removed, contacts cleaned, replaced and tested in accordance with the applicable calibration type surveillance test.

Failure Data:

No previous failures of this type.

Very truly yours,

?
