Refuel Area Exhaust Radiation Monitor, E15-3-17-45CC, failed downscale.
Conditions Prior to Occurrence:
Reactor at full power.
Description of Occurrence:
During norr.s1 steady crate operation, the
'C' refueling area exhaust radiation monitor failed downscale.
Designation of Apparent Cause of Occurrence:
The apparent cause of the occurrence la under inecatigation at this tine.
Analysis of Occurrence:
Eecause this device is one of four arranged in a one-out-of-two twice Io; tic (which initiates a secondary containment ventilation systen isolatien on refueline, fIcor exhanst high radiation) such that the Icss of any one device will not deicat the logic, the safety isp11 cations are mininal.
Corrective Action:
This channel was isarediately placed in the tripped condition in cosspliance with Table 3.2.D of the Technical Specifications.

Previous failures reported as 50-277-75-20, 50-278-74-2, 50-278-75-21.
