Unit 3 at 50 power.

Dcocription of Occurrence:.

During routine operation. of the TIP cachines, the

'B' detector failed.to stop nt the shield linit and close the ball velve as it was being withdrawn to the shield che-n:o Designation of Apparent Cause of Occurrence:

The chield li=it switch, which controls detector travel and ball valve cperation.

failed to operate when the detcetor resched the "in shield" position. This allowed the detector to continue ito withdrawal until it was manually stopped Analysis of Occurrenec:

The failure of the li=it switch to halt the withdreval cnd close the ball valvu vas incediately obcerved and corrected by switching to the.anual code of operation ani stopping the e.achine.

The ability to close the ball valve by turning the rachine vi (de-energice to close) was verified and would have been availcble if required.

Therefora, there are no stafety it:plications to this occurrence..

Corrcctive I.ction:.

A The detector was returned to its in-shield position. The cachine vos de-tr.ergized, r.nd the ball valve was verified to be closed.. Administrative procedures were initiated to eruture that. the TIP tuchine rc=.ains in this condition until the licit switch can be repaired..

Failure Data:

No prevacus failures of this type.
