Designation of Apparent Cause of Occurrence:

Examination indicated moisture had accumulated in the MO-10-89C j

motor, causing the motor to trip electrically. A previously repaired M0-10-89C valve stem packing Icak is suspected to have caused the water accumulation in i

the motor.

Analysis of Occurrence:

There were minimal safety implications associated with this failure.

The valve is used only in the shutdown cooling or torus cooling mode of operation, neither of which is required with the unit at power. As well, the valve remained mechanically operable and could be manually positioned as required.

. Corrective Action:

The motor was dried out and returned to service af ter proper operation was verified.

i Failure Data:

Previous failures reported as A0-277-75-24, 278-75-5, 278-74-36, j

277-74-13, 278-74-17, and 277-74-53 Very truly yours?,
