Unit 3 reactor at approximately 49.6%.

  • cscription of occurrencet During the performance of Surveillance Test ST 6.11, the RCIC pump suction valve, MO-3-13-18, failed to operate electrically from the main control roon switch.

The valve was in the opened position.

tenignation of Apparent Cause of Occurrence:

Investigation deternined that the positive d.c.

lead to the control relays had pulled away frcu the terminal strip hold down screw.

.ualysis of Occurrence:

The RCIC systen is required to =ake up to the nuclear systen when the nain condenser in unavullabic er when the feedvater syste= in unavailable.

Although this RCIC synten vould not have auto =atically transferred its cuction from the condensate stornge tank to the torus on low condenante storage tank level, it would have supplied water from the condensate and is backed up by the HPCI nysten which was operable.

Safety significance of this occurrence is, therefore, minimal.

l Tne Icad to the control power relays was connected securely and the valve j

van stroked several timen satisfactorily.

Repairs were co=pleted within a few hours of the occurrence.
