Description of Occurrence.

The bypass valve was inadvertently Icf t open for 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, thereby removing the particulate and iodine sample filters from service for this period of time.
Designation of Apparent Cause of Occurrence:
Individual error is not closing the filter bypass valve after draining the moisture from the line.
Analysis of Occurrence:
During the 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> period of time that the filter and cartridge were out of service, Unit 3 was running at a constant power le ve l. The main stack gas n.onitor was in service during this period of time. The gas release for this period of time varied between 70 Ci/sec. and 230 Ci/sec. (pe ak). Therefore, the percentage of Technical Specification limit from the main stack was (6 7X10-25 (3...C.1).
Since the iodine 8
and particulate release rates usually follow the gas release rates, it is unlikely that any significant release of iodine or particulates occurred during this period.
Corrective Action:
Filter cartridges were replaced and the filter bypass valve was closed. An instructional tag was affixed to the valve, cicarly indicating that the valve is to be kept closed except when cartridges are being changed.
