Failure of stack flow transmitters.

Condi tions Prior to Occurrence:

led Mr. A. Giambus 50-1 75-54 & 50-278-75-44 July 30, 1975 Page 2 Designation of Apparent Cause of Occurrence:

Lightning strike.

Analysis of Occurrence:

Flow rate up the main offgas stack during normal operation is provided by dilution fans.

Two dilution fans were in service prior to the lightning strike and remained in service during the entire period that the flow transmitters were out of service.

Therefore, total offgas stack flow is known.

This permits an accurate deter-mination of total gaseous release during this period. This occurrence has little safety significance.

Corrective Action:

An inspection in the area below the main stack was performed immediately following the failure.

Repair of the electronics was ini tia ted.

One of the two flow transmitters was returned to service.

Repai r of the second transmi tter is awai ting replacement parts.

Failure Data:

Previous failures of this type were reported as 50-277-75-51 and 50-278-75-38.

Very truly yours, M.
