Dear Mr. Stello:
Subject:
Environmental Deviation The following deviation was reported to Mr. Walt Baunack, Region 1, Office of Inspection and Enforcement, United States Nuclear Regulatory Comission on May 4,1976.
Reference:
License Number DPR-44/56 Environmental Technical Specifications:
2.1 Report No.:
50-277/278-76-25 Report Date:
May 14, 1976 Deviation Date: May 4, 1976 Facility:
Peach Bottom Atomic Power Station Units 2 and 3 R.D. 1, De l t a, PA 17314 Identification of Deviation:
Three cooling towers simultaneously inoperable.
Conditions Prior to Deviation:
Unit 3 operating at 56% power. Unit 2 off for a refueling outage. The "A" and "B" cooling towers out of service due to previous lift pump bearing failures. River flow was about 33,000 CFS.

Description of Deviation:
The "C" cooling tower was removed from service for 9 hours1.041667e-4 days
0.0025 hours
1.488095e-5 weeks
3.4245e-6 months
, making three cooling towers inoperable.

50-277/278-76-25 May 14,1976 Paga 2 Designation of Apparent Cause of Deviation:
The "C" tower lif t pump was inadvertently tripped during bus switching. The lif t pump breaker would not reclose.
Inve stigation showed a flow switch indicating loss of lube water to the lif t pump had become inoperative. The indication was caused by accumulation of sitt in lubricating water lines.
Analysis of Deviation:
Differential temperatures between the Pennsylvania-Maryland State Line and Holtwood Dam remained within Pennsylvania State limits.
Single unit operation and the short duration of the outage minimized the thermal impact of this deviation.
Correcti ve Action:
Lubricating water lines were flushed and the "C" lif t pump and tower returned to service on 5/4/76.
Failure Data:
Several previous occurrences.
Very truly yours,
/
M.
