SERVICE WATER ELECTRICAL DUCT RUN OMISSION OF REBARS NO. 1 AND 2 UNITS SALEM NUCLEAR GENERATING STATION On January 24, 1973 we'were advised that two concrete pours for the service water electrical duct run between the fuel handling and auxiliary buildings were placed without-installation of specified robars. This deficiency was.
reported to the U.S. Atomic Energy Commission, Directorate of Regulatory Operations,fRegion 1 3 on January 26,=1973, one day delayed because of the intervening Federal holiday on January 25.
It is standard practice to place concrete ~around-conduits in duct runs on the basis of information specified on the electrical conduit drawings. .Due to the Seismic 1-
- clacsification'of this electrical duct structure,-a structural-drawing showing rebar placement.was prepared'at a later date from.the elec*'ical' conduit-drawings.' On' subsequent. review by'the field =ces of structural drawing' 228547- A-1322-1 and -
related docu- ,ation,-the nonconformance was found..
These two_ concrete pours will':Lbe removed and will be: replaced in.'accordance with details-specified on
' drawing 228547-A-1322-l' " Service Water Intake Yard Duct. -

U.S. Atomic Energy 2-27-73 Commission To prevent a recurrence of this type of noncon-formance, the job engineer will hold strict adherence to Field Procedure FP- 5 "Manufa c ture , Placement and Curing of Concrete,"
which provides for a review of applicable documents prior to pouring concrete. To further assist in this review for applicable documents, the Designing Division has been instructed to cross reference electrical and structural drawings when these drawings are complimentary.
