MAIN STEAM RELIEF MANIFOLD '

NDE - NONCONFORMANCE ,

NO. 1 UNIT SAIEM NUC LEAR GENERATING STA' LION DOCKET NO. 50-272 On December 6, 1974 a verbal report was made to Region 1, Directorate of Regulatory Operations that three main steam relief manifolds for No. 1 Unit, Salem Nuclear Generating Station had been received and of which two were partially installed when it was discovered that the welded branch connections had not been radiographed. This report was made in accordance with provisions of 10CFR50.55(e).

The three manifolds delivered are identified as MS-7, .MS-14 and MS-21. During the installation of MS-7 and MS-21 difficulty in fit-up due to out-of-roundness led to a stopu s hipment order to the vendor for the fourth manifold (1-MS-28) to permit shop inspection and repair. The extent of the repairs made*to 1-MS-28 were sufficient to require an additional furnace post veld heat treat-ment. In reply to the vendor's request to permit waiving the require-ment that each shop weld be radiographed following post weld heat treatment, PSE&G advised that it was their opinion that repeat radiog-raphy following heat treatment was not necessary and that the post weld heat treatment would have no deleterious effect on those welds which had been previously radiographed and accepted.

On the repeat review of the prior radiographs the shop code inspector accounted for all the butt welds in accordance with B31.7 but found that there were no radiographs for the welded branch' connections between the weldolets and manifold body. In October 1974 work on the manifolds was stopped in both the field and in the fabricating shop and the vendor was directed to perform radiographic examination of the welded branch connections on all of the subject manifolds. The examination disclosed unacceptable indications which require repairs to quality for PSE&G acceptance. Manifolds MS-7, MS-14 and MS-21 will be returned and repaired in the vendor's shop.

The investigation conducted by PSE&G, which included an in-depth audit of the vendor's fabricating facility, disclosed that the vendor had made a misinterpretation of B31 7 code, Table A.7(b) as related to PSE&G Specification No. 61-6200 for Main Steam Piping, Schedule No. IS12, Group A. The shop instructions called for
radiographs for all butt welds, but erroneously omitted the I radiography requirements for the branch connections. Shop instruc-tions called for magnetic particle examination of the welded branch connections and the surveillance by QC was on the basis of those instructions.

Our current audit of the vendor's shop facilities disclosed that his present quality program has been upgraded so as to preclude the likelihood of similar errors in the future.

PSE&G has also reviewed other related work documents to assure full compliance with codes and specifications. The PSE&G Vendor Surveil-lance Representative and the site receipt inspection personnel have been directed to assure themselves of properly approved and correctness of vendor initiated travelers, sketches, or other QC instructions prior to use. To further assure the performance of vendor surveillance resident representatives, more frequent visits of the vendor's facilities will be made by the Supervising Engineer-Vendor Surveillance.

It is estimated that the repairs to the subject welds on the Main Steam Relief Manifolds will be completed by June 1975 Detailed documentation covering this work will be available at the site for your future inspection.
