Dear Dr. Knuth:

~

CASK HANDLING CRANE AUXILIARY HOOKS NOUDESTRUCTIVE TESTING NO. 1 AND 2 UNITS x SALEM NUCLEAR GENERATING STATION .

a#

DOCKETS NO. 50-272 AND 50-311 On January 23, 1975 a verbal report was made to Region 1, =

Directorate of Regulatory Operations that two Auxiliary. Hooks 4 -

(3-ton capacity) of the Cask Handling Cranes for No. 1 and "

2 Units, Salem Nuclear Generating Station were inspected in accordance with NJAC 12:1h8, Section 4, " Inspection, Testing i and na unenan c e vi Overhead Gantry Cranes," and were round

! defective. This report was made in accordance with provisions '

of 10CFR50.55(e).

The Cask Handling Crane and Auxiliary Hooks were purchased from

' Whiting Co rporation, Harvey, Illinois. The serial numbers of the two 110-ten cranes are 10255 and 10256. The rating of the auxiliary hooks is three tons. The raw material (ASTM A351-70 GR CF8) for the auxiliary hooks was supplied by Oklahoma Steel ^

Casting, Tulsa, Oklahoma. An ultrasonic test was performed on ==

the cast auxiliary hooks by Clark Inspection Service. Inappro-priate standards of acceptance were used and no defects were'

reported in the material. .

The required annual crane inspection per "NJAC 12:148" was ~~7 carried out by Eastern Testing & Inspection, Inc. for PSE&G.

Since the hooks were cast of stainle ss steel, the specified  ;

magnetic particle test could not be performed and an alternate test was carried out in accordance with ASTM E-165, " Methods for Liquid Penetrant Inspection, Method B, Visible Dye Penetrant."

The dye penetrant examination results revealed several linear -

indications in the auxiliary hooks of both No. 1 and 2 Unit cranes. -

The test results pointed to a problem of major concern requiring further examination by radiography. The radiographs which were __

taken of No. 1 and 2 Unit auxiliary hooks showed that several of the previously identified linear indications were of such magnitude ,

to be evaluated as cause for rejection of the auxiliary hooks.

PSE&G Engineering Departner.t sponsor engineer has evaluated these findings and is in the process of obtaining replacement auxiliary hooks from Whiting Corporation. Carbon steel hooks of other cranes which were tested -in accordance with NJAC 12:148 were found to be free of similar indications.

Acceptance criteria for the replacement hooks will be specified by Whiting internal documents. Finished hooks shall be load tested to 125 percent of rating after which they shall be sub-jected to a liquid dye penetrant test and radiography for acceptance evaluation prior to shipment.

Detailed documentation covering this activity will be available at the site for your review. If additional information is needed, we will be pleased to confer with you.

Very truly yours,

/ fwf -
