Dear Sir:
In response to the violation which was reported in the subject inspection, we offer the following:
NRC Notice of Violation (443/83-13-01):
10 CFR 50, Appendix B, Criterion V and Section 17.1.1.5 of the Seabrook Station FSAR require that activities affecting quality be controlled by and accomplished in accordance with the appropriate instructions, procedures, or drawings. United Engineers and Constructors (UE&C) Administrative Procedure (AP-15, Revision 17) specifies the program of controls for changes to design documents and requires that major generic Engineering Change Authorizations (ECA) be issued for implementation only af ter design concurrence is accomplished.
Contrary to the above, as of August 1,1983, a major generic ECA (19/1591A), a revision to the torque requirements for the cooling tower service water piping flange connections, was issued and implemented without design concurrence having been accomplished.
This is a Severity Level V Violation (Supplement II).

Corrective Action Taken and Results Achieved:
This ECA was written as a standard Major Generic ECA and was inadvertently released to the installation contractor (P-H) for implementation prior to UE&C Home Office concurrence. When it was determined that it should have been handled as a Specific ECA, the standard ECA was removed from the system (before formal issue) and replaced with an On-The-Spot ECA.
Because of the mix up in ECA processing, P-H issued NCR #4925. ECA 19/1591 was subsequently voided and the required technical information was transmitted via the Nonconformance Review Board Report for NCR #4925.
To preclude repetition of this isolated occurrence,' UE&C has taken the following steps:

A management memo has been issued to UE&C Site Engineering staff l
reinforcing the requirement for concurrence of Major. Generic ECAs before issuance to contractors as required by Project Administrative Procedure 15.

The issue of prematurely initiating work with the contractor prior to issuance of ac ECA has been discussed with the responsible engineer. Any l-previous misunderstandings have been clarified.
3.
The Design Change Program is -structured so that a Specific ECA supersedes a Generic ECA. Contractor programs are structured for compliance with a Generic ECA when it is applicable. YAEC has requested that P-H clarify their Procedure IX-6 stating that Specific ECAs supersede Generic ECAs and design guidelines.
Corrective action, including necessary procedure revisions will be completed by November 15, 1983.
