SEQUOYAll NUCLEAR PLANT (SQN) - UNIT 1 - FACTLITY OPERATING LICENSE DPR SPECIAL REPORT 91-14 This special report provides details concerning two fire barriers being breached for greater than the Lt2hnical specification allowable time period. The auxiliary building Fire Door A-62 has been breached to facilitate traffic during the Unit 1 Cycle 5 refueling outage. The

! biological shield blocks have been removed, and the Unit I reactor building equipment natch has been opened to provide unimpeded access during the refueling outage. This report is being made in accordance with Technical Specification Action Statement 3.7.12. The enclosure contains the details concerning the condition.

t Description of Condition On-October.4, 1991, with Unit 2 operating in Mode 1 (100 percent power, 2235 ponds per square inch gauge, and 578 degrees Fahrenheit), and Unit 1 being shut down for_a planned refueling outage, auxiliary building Double Fire DoorLA-62-was removed from service to facilitate outage activities associated with personnel traffic entering and exiting the Unit-1 penetration room on Elevation-690. The fire door will be nonfunctional for the duration of-the refueling outage.

On' October"5, 1991, the Unit i biological shield blocks were removed and the Unit i reactor building equipment hatch was opened to provide access during the refueling outage.

Cause of tne Condition Auxiliary building Fire Door A-62 is a double door, and half of the door opening will be roped of f for a contamination zone (C-zone) corridor to allow-

-personnel access'ta the hot' tool room constructed outside the penetration room.- The location of the hot tool room is normally outside the C-zone. .This-C-zone corridor will eliminate time lost to exit the C-zone areas to obtain tools and will eliminate the additional radiological waste associated with the C-zone exits.

The shield blocks were removed to permit-the Unit 1 Cycle 5 refueling activities.

Corrective Action A breach permit was issued in response to the work request to remove double p Fire Door _-A-62 from service. The fire detectors inside the Unit 1 penetration-L room were verified operable. .A roving fire watch was established and will be l maintained until the door is returned to operable status and the-breach is removed-at the end of the cutage.
