On May 1,1995, Channe 1, Lower Reactor Vessel Tube 4, was declared inoperable. The channel was evaluated on May 10,1995 with no problem found. Data from May 23,1995 ] 4 also indicated that the channel was operating as expected by a healthy channel. However, the channel was not removed from the Technical Specification Action Item Log (TSAIL) and declared operable. Channel 2, Lower Reactor Vessel Tube 9, is operable and available to cover the collection region.

Since Channel I has been inoperable for more than thirty days, it is reportable to the NRC per Technical Specification 3.3.3.9. This report was due to the NRC on June 12,1995; consequently, this report is being submitted eight days late.
