In response to the violationt which were reported in the subject inspection, we offer the following:
NRC Notice of Violation (443/8')-08-01):
A.
10CFR50, Appendix B, Criterion XIII, requires that measures shall be established and performed to prevent damage or deterioration during storage and installation. United Engineers and Constructors Procedure GT-M-102 and Limitorque storage procedure, dated August 13, 1976, requires that during installation of the Motor Operator Valves (MOVs), that the limit switch compartment must be maintained clean and sealed to minimize the ingress of foreign materials during and af ter installation work in the limit switch compartment area.
Contrary *o the above, on May 25, 1983, MOV-CBS-V-0003 and MOV-CBS-V-0005, installed in the equipment vault area, had the limit switch compartment unsealed and foreign material on the electrical components within the compartment.
This is a Severity Level V violation (Supplement II).

RESPONSE
Corrective Action Taken and Results Achieved:
At the time of this inspection, the subject equipment status was classified as "in place storage". As such, it was under the jurisdiction of the "reventative Maintenance Department.
Previous to this inspection, the operator's lubrication had been changed under GT-M-102 as part of a plant-wide standardization of lubricants. Addit ionally, the operators were checked for general workmanship conditions, and the results rec orded.
Subsequent to the operator lubrication and general workmanship check, the limit switch should have been resealed per GT-M-102; however, the resealing was not accomplished. This rework is performed under the control of the Preventative Maintenance Department with UE&C Quality Assurance participation.
To prevent a recurrence, area engineers have been reindoctrinated regarding in place storage requirements and the importance of the need to be observant of preventative maintenance requirements as part of their daily activities.
The reindoctrination was completed by June 10, 1983.
The licensee wishes to clarify that items categorized as "in place storage" are controlled by the Preventative Maintenance Department and, as such, are subjected to periodic, scheduled maintenance checks. UE&C QA personnel participated in these ace.ivities.

United Engineers and Constructors Electrical Installation Specification Number 9763-006-48-2 requires that, during i
installation, cable trays shall be clean and free of debris, tools, etc., and maintained during the construction phase, Contrary to the above, on May 25, 1983, the safety-related cables t
and raceways in the Primary Auxiliary Building and the Equipment Vault area had paper, wood, tie wires, and a metal can in the Tray Systems.
This is a Severity Level V violation (Supplement II).

The following actions have been taken to improve site cleanliness in general, and cable trays in particular:

A Project N tice was distributed by the Project Construction o
Manager, emphasizing the need to keep cable trays free of construction debris and other unauthorized meterial.
2 The project-wide System Cleanliness Program was revised to incorporate actions required by specific contractors.
3.
FBM Procedure FECP-5004 (Construction Procedure for Installation of Safety and Non-Safety-Related Cable) is being revised to indicate the requirement for cable trav cleanliness.
4 FBM has established a cleanup crew on the second shift.

UE&C In-Place Inspections will denote cable tray cleanliness problems.
6.
General Housekeeping Inspections (UE&C Procedure FGCP-8) will address cable tray cleanliness.
7.
Retraining of contractor supervisors, emphasizing the requirements in the System Cleanliness Program.
Implementations if these actions takin must be an on-going process, and the results wili be monitored by YAEC.
