Dear Mr. Keppler:

This letter serves as an interim report pursuant to 10CFR50.55(e) on any potential damage which resulted from the(fpilure to follow procedures during an attempt to remove the Unit 1 Shroud Head / Separator from the Reactor Pressure Vessel by the use of a strongback and polar crane 2 Mr. P. R. Pelke of your office was first notified on September 16, 1983, by Mr. E. Riley of The Cleveland Electric Illuminating Company (CEI) that extensive evaluation will be required to assess the significance of any damage.

This report contains a description of the incident which resulted in this potential significant deficiency, identifies the equipment being evaluated, outlines the steps being taken to determine the extent of the damage, and corrective action requested.

Description of Incident On September 15, 1983, a Perry Nuclear Power Plant (PNPP) contractor was attempting to { remove the Unit 1 Shroitd Head / Separator from the Reactor Pressure Vessel (RPV) by the use of a strongback and the Polar Craneg The attempt ceased when the strongback suffered damage. The damage was caused because the contractor had not removed the studs holding the Shroud Head / Separator to the Shroud flange. This was contrary to procedural requirements.

Mr. James G. Keppler October 12, 1983 Actions to Determinc Extent of Damage CEI contacted the Polar Crane vendor, P&H Harnischefeger, to notify them of the patential overload on the crane and requested their recommendations on items to check for damage. None of the inspections recommended by P&H Harnt-schefeger have shown any damage to any part of the crane. As a precaution we replaced the potentially overloaded wire cable with a new certified cable.

The crane has been tested to a lower weight initially to remove the Shroud Head / Separator from the RPV.

It will be retested using recammendations from P&H Harnischefeger.

CEI also contacted the RPV, Shroud Head / Separator, and strongback vendor, GE-NEBO, for recommendations on items to check on the Shroud Head / Separator and RPV for damage. The Contractor has brought an exact duplicate strongback from a cancelled nuclear plant and has load tested and certified it for use at PNPP. We have performed inspections in accordance with all GE-NEB 0 recommenda-tions and have found no rejectable items. Further testing on the RPV and/or Shroud Head / Separator will be required by GE-NEB 0 af ter the Shroud Head /

Separator has been removed from the RPV. CEI has requested that GE-NEB 0 do an analysis on the accident to determine as accurately as possible the load required to damage the strongback and whether that load might have any affect on the RPV internals. We will also supply the load to P&H Harnischefeger for their records.

Corrective Action The site contractor has been directed to revise his procedures to require sequencing documents, briefing, and rigging sketches prior to lifting items such as the Shroud Head / Separator.

Because further evaluation and testing will be required on the Polar Crane, Shroud Head / Separator, and RPV, another report will be issued January 31, 1984.
