During the Unit 2 refuel outar,e that began on September 11, 1991, a number of fire barrier penetration Limiting Conditions For Operation (LCO) were initiated.

Four of ther.e LCOs ercaded their seven day time frame They are as follows:

At 08')0 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> (EST) on September 19, 1991 an LCO was initiated due to removal of the north hatch on the Emergency Core Cooling System Mini Steam Tunnel Roof, located in the Unit 2 Reactor Building. This LCO was initiated to facilitate outage related plant modification work Appropriately stationed f tre watches were established at that time.

Due to the scope of the work, the seven day time frame of the LCO was exceeded -

on September 26, 1991. The LCO was canceled on October 12, 1991.

Another LCO related to the above mentioned plant modification work was initiated at 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br /> (EST) on October 4, 1991, for fire door no. 206. This door is located in the Unit 2 Reactor Building, 23' elevation. Appropriate fire watches were established.

Due to the scope of the work, the seven day time f rame for the LCO was exceeded on October 11, 1991. On October 12, 1991, door no. 206 was restored to service and the LCO was canceled.

On October 4,1991, at 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br /> (EST), two LCOs were f aitiated for the opening of penetrations R2 2-008 and R2-2 009. These penetrations were opened to facilitate running cable 1 cads into the Unit 2 Reactor Building from the Turbine Building Breezeway f or outage-related activities. Appropriate fire watches were established. Both LCos exceeded the seven day time f rn'ne on October 11,1991. The work for which the seals were opened was completed on October 24, 1991 and at 0530 the LCOs were canceled. Until restoration of the seals and cancellation of the LCOs, appropriately stationed fire watches remained in effect.
