Browns Ferry nuclear Plant unit 1 Identification of Occu*rence HPCI failure to start.

Conditions Frior to Occurrence The reactor was at approxi=ately 95-percent power.

Description of Occurrence During routine surveillance testing, HPCI. failed to start on the first attempt.

j The run light for the auxiliary oil purp was on, but the stop valve failed to open. Tne turbine low bearing oil pressure alarm was received and would not 1

clear until the system was shut down. An operator was dispatched to the HPCI

room to inspect the systen. After insnecting the.cysten and finding nothing i

abnormal, he started the auxiliar/ oil purp locally; and the stop and control valves opened satisfactorily.

Tne surveillance test was again initiated and HPCI performed sat,isfactorily.

a Designation of Armarent Cnuse of Occurrence The movabic contact of the auxiliary oil pump rotor contactor was found to be dragging against the inside of the are chute.

It is believed this caused the contact to hang preventing the auxiliary oil purp motor from starting.

Analysis of Occurrence 1

The failure of the auxiliarf oil pu=p to start on automatic initiation caused HPCI to be inoperable. There was no da=cGe to systems, co=ponents, or structures as a result of HPCI failure to start. There was no jeopardy to reactor safety as other engineered safeguard cystens were operable during the period HPCI was

inoperable. There were no adverse effects on the health or safety of the public i

as a result of this failure, o

Corrective Action J

The movable and stauionary contacts were elecned and realigned. The contactor was operated several tires to ensure proper clearance between the novable contact and the are chute. Tne auxiliary oil purp was started several times, and the contactor operation was normal. Operation of the stop and control valves and the-oil systen was observed to ensure proper operation.

The oil pressure was nortal and the stop and control valves operated satisfactorily. The unit 1 HPCI auxiliarf oil pump notor contactor was inspected and found to be functioning satisfactorily.

Failure Data The contactor is a Genercl Electric Company contactor, catalo6 No. 1C2800-1617L2H.

The coil is for 230/250V d-c operation.
