Report Date: November 11., 1974 Occurrence Date: Novc=ber 4, 1974 Facility: Browns Ferry Nuclear Plant unit 2 Identification of Occurrence The HPCI system was inoperable as a result of blown head gaskets on the gland steam condenser and flooding of the gland steam condenser hotwell purp motor.

Conditions Prior to Occurrence The unit was in a hot shutdoun condition following a reactor scram.

Description of Occurrence The HPCI was autonatieny initiated following a reactor scram at 0938 hours0.0109 days <br />0.261 hours <br />0.00155 weeks <br />3.56909e-4 months <br /> on Novc=ber 4,1974 The water level in the reactor was restored with no indication of trouble. A short time later, the HPCI room was inspected and no leaks were noted. At 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br />, grocnd indication was received on the gland steam condenser hotwell pu o motor. An investigation revealed that leakage from both the upper and lower head gaskets on the gland steam condenser had flooded the gland steca condenser hotwell pu=p and co+or.

Designation of Annarent Cause of Occurrence The exact cause has not been determined. -

Analysis of Occurrence Failure of the head gaskets on the gland steam condenser which caused the HPCI to become inoperable did not jeopardize unit operation or create a hazard to the public. The other core cooling systens were operable.

Corrective Action

1. The upper and lower head gaskets on the gland steam condenser were replaced.
2. The gland steam condenser hotwell pu=p =otor was dried out and meggered s satisfactorily. "
3. The HPCI was tested and returned to operable condition. .
4. A plant modification has been initiated which incorporates General Electric Co=pany's corrective action reco=nendations including provision of a metal cln ping ring with a gasket to support the present gasket and lowering the inlet cooling water supply relief valve setpoint. These recommendations will be considered by TVA Design and implemented when approved.
