MONTH!.Y OPERATING REPORT - SEPTEMBER 1991 At the beginning of September, Ofster Creek was operating at. about 70% power while t, condensate pump was repaired, full power was achieved on September 4th.
On Septernber 25th, during performance of tha isolation Condenser isolation test and calibration surveillance, pressure dam)ers in the instrument lines failed to meet their pressure bleedoff criteria. Due to the potentially degraded condition of the isolation condensers, a plant shutdowr, consnenced in parallel with repairs to the dampers.
Repairs to t.he dampers scre completed, the surveillance completed satisfactorily, and the plant
' rned to full power on September 20th.

On August 22, 1991, at 0306 hours0.00354 days <br />0.085 hours <br />5.059524e-4 weeks <br />1.16433e-4 months <br />, a reactor scram occurred due to low reactor l
water level.
The cause of the low reactor water level was a loss of feedwater flow due to a main feed pump trip on low suction )ressure.
The root cause of this event was a grounded condensate pump motor wiish led to a condensate pump trip while the plant was operating above 80% power.

The shrink in reactor water level caused by the scram and the reduction in feedvater flow caused reactor water icvel to reach the double low Reactor Protection System (RPS) setpoint which actuated appropriate Engineered Safety features.
A sncond event being reported is a procedure violation which occurred at 1012 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.85066e-4 months <br />. While plant cooldown to cold shutdown cohditions was in progress, all five reactor recirculation discharge valves were shut simultaneously with reactor temperature above 212*F.
