Dear Mr. O' Leary:

This letter is written to provide a preliminary report of an aMomal occurrence (No 74-14) which occurred at the Palisades Plant on July 11, 1974. This abnormal occurrence report involves High-Pressure Safety Injection Pump P-66 A.

During quarterly testing of the high-pressure safety injec-r.mn pump, vna enu+ e rr d_ie h nr : pr:::: : , ,: :, friciently t ::, re,te,i to raise the question of possible pump degradation. Shutoff during the original manufacturer's test was 2,850 feet of water while during

- the quarterly test the shutoff head was observed to be 2,537 feet of of water.

A pump head test was perfomed on July 19, 1974 to' determine if pump flow characteristics had been effected. This test revealed that the actual pump head iras less than the manufacturer's head curve throughout the operating range.

The pump vill be disassembled and inspected to determine the cause of the flow de$radation. This disassembly and inspection vill be completed prior to returning the plant to service. An abnomal occur-rence report vill be submitted in the fomat described in Regulatory ,,

Guide 1.21 after completion of these investigations and prior to re-turning the plant to service.

This abnomc1 occurrence has been acsi6 ned No 74-14. The previous abnomal occurrence report submitted in this format was 'as-nigned No 74-12. For plant record purposes, the events described in the report entitled " Palisades Plant Waste Gas Handling System" have been asnigned Abnormal Occurrence Report No 74-13. Thisrepopwas ,
