"B" RESIDUAL HEAT EXCHANGER LEAKAGE

Dear Mr. Block:

In accordance with Paragraph 6.6.1.c, Technical Specification, the following abnormal occurrence is reported:

On May 8,1971e ith No. 2 Unit being maintained in a cold shut-w down condition utilizing "B" RHR pump and heat exchanger, an alarm was received on Radiation Monitor, R-17, located in the component cooling system.

At the same time, the component cooling surge tank level began to increase.

R-17 counts increased from 70 CPM to 1.75x10 CPM over a twenty-five minute period and the surge tank level increased approximately 25% during this period.

The "B" heat exchanger was isolated and activity in the component coding system leveled off at 1.75x105 CPM. As this is a closed system, no activity was released to the environment and the plant can continue to function in its present condition with no danger to plant personnel.

The heat exchanger will be inspected and repairs will be made.

In accordance with paragraph 3.3.1.1.f, the plant will not be made critical until repairs are complete.

This occurrence was reported to AEC Division of Compliance by telephone and telegraph on May 8, 1972.

In accordance with Section 6.2 of the Technical Specifications, this occurrence was reviewed by the Plant Safety Committee on May 9, 1972.

A report will be prepared per Section 6.2.2 and reviewed in accordance with 6.2.3.
