I!M //18 detcetor was removcd on 10-16-74 to replace the drive tube. The de-tector wau reinstallcd in the new drive tube and was found to bc inopcrative.

On 10-17-74 the fuel handlers atterptcd to rcmove the faulty detector when it became stuck in the schuttle tube. The dctcetor was unable to Tech be rc=cved Specs

,and was left in the core cince it is within its norcal boundry.

do allow one IIDI per channel to be bypasscd.

APPA RENT CAUSE OF UO.!SUAL EVF1.T:

It Equipment failure - the IE4 detcetor bccarne stuck in the schuttle tube.

is pocsible that the ccupling between the schuttle tubc and the drive tube was over tightencd and causcd the o-ring to be forecd into the tube arca.

This may have cauccd a rcstriction schich cauced the insulation on the dc-

  • tector to become torn and bound when installing the dctcetor.

ANALYSIS OP UNUS"AL E'.T.7:

Safcty of the reactor is not affceted. Houever, if another IE4 in the same channel.becoces incperative Unit 1 will have to operate in the IEi region with a half-scram. Scven of the eight ICI's are operational at this time.

, CORRECTIVE ACTION:

A new schuttle tube has been ordercd. When the schuttle tube has arrived the IIM and its associated schuttle tube will be replaced during cnout.ag'c on Unit 1.

- FAILURE DATA:

This is the first time that this has happencd at QCNPS, thcrefore, there is no failure data available at this time.
