Dear Mr. O' Leary:

This letter is written to apprise you of a malfunction that occurred June 5,' 1972, at the Palisades Plant pertaining to the failure of Control Valve (CV) No 1807 to fully close.

This h8-inch butterfly valve, that normally is in the closed position, is located in the discharge line of the containment air purge supply and is one of two valves that provides containment isolation.

These valves vill close from a hand switch operated in the control room, a containment high-pressure signal or a containment high-radiation signal.

Plant personnel were securing the purge air system when this valve failed to close past the 90% open position. Several attempts vere made to operate the valve from the control room hand switch with no response. The packing glands on the valve were backed off and lubricant was injected into the packing area. The valve operation then returned to normal. This set of valves was leak tested very recently and a pack-ing adjustment was made at that time. The valve was test operated after the leak test and there were no abnormalties noted. .

We have concluded that the valve would not close past the 90%

open position because the packing gland was tightened sufficiently to partially bind the valve operating shaft. After the valve had set in the open position during the recent outage, the packing then took a fixed set and would not allow free movement of the valve stem.

This valve is ncy fully operable and to insure operability, a daily test procedure has been instituted for the next two weeks. The redundant valve CV-1608 was test operated and no abnormalties noted.
