Subjects Dyster Creek Nuclear Generating Station Docket No. 50-219 Inspection Report 90-23, operator Status Reference 16 Inspection Report 90-23 Reply to Notice of Violation dated February 25, 1991 I
The purpose of this correspondence is to supplement the corrective actions l
provided to you in Reference 1.
In conversation with your staff (Mr. William Ruland), it was determined that our response to the Notice of Violation did not address the control of administrative directives which appeared to contribute to this event.
In this event, a directive was issued but was not incorporated into a procedure. Normal practice is to l
proceduralize these directives if they are long term, continuing directives; i
however, in this case, the dirschive was not proceduralized. The below listed corrective actions will assure that personnel entering new positions are awara of directives and policies established prior to their coming to that position.- In order to preclude a recurrence of this instance, the corrective actions itemized below will be implemented by 9/1/91.
Long_ term policies and/or directives will now be emptured in procedures or other appropriate long term controlled documents (such as qualification cards, etc.).
Department-procedures 106, 106.4, and 106.6 will be revised to reflect that long term policies / directives be captured in procedures or other appropriate long term documents.
