High Pressure Coolant I.ajection (HPCI) steam line floa switch, FS-2-2352, r failed to cause an isolation of the system wiihin Technical Specification limit.

The unit was'in the cold shutdown condition fer a refueling outage.

, DESCRIPTION OF OCCURRENCE:. <

On February ,8,19'15,T.tfie doing' routine surveillance testing of the High Press 6re Coolant injection System (HPCI) steat., line flow switches, FS-2-23{2, fiiled to irip-ac a6y applied dif ferential pressure. The purpose of'this swi tch is to isolate the HPCI system when the steam flow I'st 300% of the 'rateu flow.". e

, DESIGNATION OF APPARENT CAUSE 0F 6fCURRENCE:

Misalignment The failure was investigated and it was discoveree! that the switch

-j linkage was binding and would not move through Its full travel.

Due to the redundancy of the circuitry, safe HPCI and thus safe reactor operation were not Jeopardized. No hazard to the health of the public or plant personnel was presented.

! CORRECTIVE ACTION:

! The linkage was adjusted and the switch was recalibrated and functionally l

tested to verify its operability.

! Range -200" to +200" of water This is the first HPCI switch which has failed by the linkage binding at Quad-cities Station; therefore, there are no safety implications based on cumulative experience.
