1 Update of Comm'tment Status.

in our original response to Violation 413.414/94-24-01 we indicated specific corrective actions would be taken to l

presrnt recurrence. Included in that response were two additional corrective actions to address the broader issuc t I-identified in the NOV cover letter which suggested increased management attention was warranted in the area of l

equipment failure troubleshooting / root cause determination.

The intent of these additional corrective actions was to impicment an imprwed Failurc 1mestigation Process (FIP).

Ji A longer term enhancenwnt was to develop a consistent, thorough, and uscabic ficid troubleshooting procedure.-

The purpose of this letter is to pro ide the status of completion of the corrective actions. The specific corrective ~

l actions as stated in our original resmnse have been completed. ' The FIP has been impicmented at Catawba for -

a equipment failure root cause imest gations. Implementation of FIP has significantly improved the quality 'of.

troubicshooting and root cause evalua6ns..

With the impicmentation of the specific corrective actions and implementation of the FIP process, uc feel that we have taken suflicient corrective actions to nxet the intent of our original response.

Enhancements to the troubicshooting procedure have been completed. Completc implementation of this procedure will require an Instrument and Electrical (IAE) maintenance task change for the Employec Training 'and Qualification System (ETQS), final review and retraining of all affected maintenance workers. All training necessary l

to complete the implementation of this procedure will be completed prior to October 1,1995.
