This supplements previously submitted Licensee Event Report No. BFAO-50-259/76hw which reported the f;iilure of a General Electric Company type SBM switch in the, RCIC system.

All GE type SBM switches on units 1, 2, and 3 were. inspeeted. There were 138 suitches f,ound to have been manufactured between July 1972 and May 1975 This is the period specified by GE in the Service Information Letter (SIL)

No. 155 during which the switches Lexan com followers were exposed to hydrocarbons. In addition, 2'37 switches were.found conteining crazed lexen

~

cam followers and 12 switches were found containing badly cracked Lexnn cam followers. All of the ebove switches were replaced except for a few in noncritical r.pplications which are being replaced as work load end plant conditions permit.

As r precaution, 33 additional switches that vere installed within the fire

.-, s residue spreed crea and were subject to the cable fire residue were identified end replaced. Also foua.d in our storeroom stock were.7 new switches with crazed Lexen cam followers and 9 with badly cracked lexen com followers.

In ::umna:y, 420 installed switches were repleced and inst' ructions were iss0ed banning the use on SBM switches of eny cleaning solvent containing bydroenrbo is.
