Dear Mr. O' Leary:
I!y letter of August 29, 1973 reported the failure of a notor operator of the concentrated boric acid 31 p feed stop valve (MOV-21h0) at the Palisades Plant. At that tira the cause of the failure was suspected to be that the valve (hence, the lever) had been used as a convenient stepping point during conatruction er.d early operation of the plant. Further investigation has revealed that use of the valve as a stepping point was not the cause of the failure. We have concluded that MOV-2140 failed becattse the can was not properly instanea auring f aci,ory uneni,1y . The w cct Asition of the ccm is on the forty-five degree bevel of the pavl as shown on the attached draving. The "as found" condition was as shown on the attached drawing. It is suspected that the loose can could get be-hind the pavl which would cause the pavls to break. This allowed operation of the valve until the nislocated can caused the pavls to break. As the cause of the failure was not due to the valve being used as a stepping point, we do not intend to pursue the elimination of stepping points as stated in the August 29, 1973 letter. We feel that the corrective maintenance performed on this valve and the inspection of two other sinilar notor-operated valves in which no abnormalities vere found provide assurance that these types of notor-operated valves vill function as designed in the future. In addition, all notor-operated valves of this type have been natisfactorily test operated as stated in the August 29, 1973 letter. Yours very truly,
