Dear Mr. O' Leary:

This letter is written to report an abnormal occurrence which occurred at the Palisades Plant on February -26, 1974, concerning the two-pump lov flow reactor protective system trip set points being found to be set at values less conservative than specified by the Technical Specifications.

The two-pump lov trip set points were found to be set as follows while performing monthly testing as required by Te:.hnical Speci-l fications Table 4.1.1, Item 3.c:

1.4 These settings were made during a January-February 1973 steam generator tube repair outage. At that time, the four-and three-pump low flow trip set points were properly calculated, documented, and set in ac-cordance with Procedure P-RP-9 It appears that the two-pump low flow set points were not cal-culated or set at that time because there was no intention of operating with only two pumps. The instrument supervisor recognized that if two-pump operation was to take place, proper low flow set points would have to be verified.

In the period between January-February 1973 and February 26, 1974, the plant had only been operated with three-and four-pump combinations.

Review of this abnormal occurrence revealed two deficient items.

These items and the corrective action to be taken is as follows:

The Technical Specifications require specific settings regardless of whether or not a particular function will be applicable to contemplated future operations. All protective system settings will be established as required by the Technical Specifications prior to the next start-up and maintained in accordance with the surveillance requirements of the Technical Specifications.

Technical Specifications Table 4.1.1, Item 3.c, requires monthly testing of the low flow trip set points.

It is not possible to test t,hese trip set points, when the plant is operating, for pump com-binations other than the operating combination and, therefore, the two-pump low flow trip set point surveillance was not performed between January-February 1973 and February 26, 1974.

A proposed Technical Specifications Change concerning low flow trip testing intervals and requirements will be submitted by April 1, 1974, in an attempt to resolve this issue to the satisfaction of both Consumers Power Company and the Directorate of Licensing.
