At the time of the occurrence, Unit 2 was at 1460 megawatts them.a1 with the modo switch in the "Run" position. The unit had an electrical load of 452 megawatts.

parameters indicatcd the valve had closed. The valve was subcoquently cycled three times and still failed to produce an alarm or half scram.

D"SIO*".?ICf 071.??r." 7 C.'.UCE 0? 000'9.RI:02 (Fabrication Error) 1.n investigation into the probica revealed that a lead had broken on the Amphenol plus from the fact acting solenoid initiating the scram function.
The apparent cauco of the breakage appeared to be that the pins in the plug were to czall for the sage wire being used.
M*AIXSIS OP 00CURRE'0E The safety of the plant and public was not in jeopardy during the failure of the number 4 control valve to givo a half scram. The fast acting solenoid j
on control valves 1,2&3 were operable and would have caused a reactor scram on fast closure of these valves.
CORRECTIVE 1.CTION The fr=:cdiato corrective ection t'as to test the valvo and issue work request nicber 10378 to inspect the cirevitry.
The emphenol plugs will be scheduled for replacement during the current E003 refueling outage depending upon the availability of materials.
