The NRC was notified of a potentially reportable deficiency. in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation. required, an Interim Report is attached. It is now expected that this information will be finalized by June 22, 1984, at which time a complete report will be i
submitted.

Potential Problem During testing, blind flanges were removed and, upon completion of the tests, the original items were not replaced. This resulted in:
Material (blind-flanges) being installed and not meeting ASME Code Class requirements nor properly identified with the original spool number (NCRs PC-8030, PC-8033 and PC-8056).
Material (blind flanges) being installed with improper spool identification (not in agreement with the documentation on file for the process line) - (NCR PC-8072).
Material (blind flange) not being installed (NCR PC-8072, item 4).
Material being removed wihout proper documentation (NCR PC-805 8).
II.
Approach To and Status of Proposed Resolution Bechtel Engineering is currently investigating this problem to determine reportability and technical justification for corrective action.
III. Projected Completion of Corrective Action and Submittal of the. Final Report Evaluation of this condition and submittal of the Final Report is-Forecast to be completed by June 22, 1984.
