Ref: Quad-Cities Nuclear Power Station Unit 2 DPR 30 . Appendix A Se ction 1. A. 2 and 6. 6.B . 3 The purpose of this letter is to inform you of the High Pressure Coolant Injection System turbine turning gear motor failure on Unit 2 at Quad-Cities Nuclear Power Station May 18, 1972 at 1: 15 p . m.

Description of Failure ,

During startup tes ts (Phase III) on Unit 2 at Quad-Cities Nuclear Power Station with the reactor at 350 psig the turning gear motor failed on the high pressure coolant in-jection system. The high pressure coolant injection system was declared inoperable at 1: 15 p . m. on May 18, 1972 and the reactor was shut down immediately. Inspection of the turning gear motor indicated burned windings.

Cause of Failure The turning gear motor has a two winding field circuit; so that the motor can be utilized for either 125 or 250 volt gj3[ direct current service. A wiring error was made which connected the field for 125 volt service on a 250 volt a('M curcuit. The winding was overloaded and burned out. The

.fK motor performed properly during testing for short periods because the ampere turns of field strength was adequate.

Corrective Action -  ;

I The turninst gear motor was sent to the General Electric j Service Shop in Bettendorf, Iowa and rewound. The motor was tested and returned to service on the Unit 2 high pressure coolant injection system on t'ay 22, 1972.
