Quad-Cities Nuclear Powei Station Unit 2 - DPR-30 Appendix A Sections 1.0.A.2, 3.2.D.1 and 6.6.A.3 The purpose of this letter is to inform you of the details regarding the failure of timer 2-1705-23 in the off-gas systen at Ouad-Cities Station Unit 2.
This incident occurred on August 12, 1972.

Descriotion of Incident The incident occurred during a Unit 2 outage with the reactor subcritical.
While trying to verify modification #201-2-72 7 f3 it was discovered that timer 2-1705-23 in the off-gas system
.i would not operate.
The timer is designed to be energized on high off-gas radiation to automatically close off-gas isola-tion valve AO-2-5406 and condensate isolation valve AO-2-5408 i
p after a fifteen minute _ time delay.

Investigation and Corrective Action Investigation indicated that one of the drive gears for
-l the motor armature of timer 2-1705-23 had come loose causing the malfunction.
A new timer was installed and e
functionally tested to operate properly.
Timer 2-1705-23 was declared operable at 2 P.M., August 12, 1972.
