The purpose of this letter is to inform you of the details regarding g '

the failure of three RCIC valves to operate ccrrectly during a monthly A ,

routine surveillance test. Valves (1-1301-26 and 1-1301-48) failed to M, ," '

close and valve (1-1301-49) failed to open. This incident occurred at

, g ,, Quad-Cities Unit 1 on September 3,1972 at 10:40 a.m. $.

At 10:40 a.m. on September 3,1972, with reactor at 50% power,-during the RCIC valves operability surveillance test, valves 1301-26 and 48 failed '

to close electrically and valve 1301-49 failed to open electrically from the control room. The RCIC system was declared inoperable and HPCI system was demonstrated to be operable.

investlantion Upon investigating the problem for the 1301-26 valve, it was determined the cause was in the 250 Vdc breaker. The opening contactor would not pull in by itself.

It was also determined, that valve 1301-43 experienced the same problem l with'the opening contactor.

Valve 1301-49 torque switch had fa.iled in the closing direction and the )

valve was tripping out on overload. z, {

By adjusting the counter spring in' the 250 Vdc breaker for valves 1301-26 and 48, the opening contactor would pull in. The valves were cycled several times and functioned properly.

The torque switch, type 0 3 was replaced in valve 1301-49 The valve was cycled several times and functioned properly. The 250 Vdc breaker was checked for spring tension and the "M" contactor coil was replaced because 'It was weak in pulling itself in.

Continuina investiaation The contactor manufacturer has been informed of the problem. A company representative will be on site to further investigate the breakers. You will be informed of any new development. .

Very truly yours, m
