Dear Mr. Knuth:

Initial report of a potential design deficiency involving weld yoke-to-motor mounting plate FCV-74-58 at Browns Ferry Nuclear Plant units 2 and 3 was made on May 14, 1974. In accordance with paragraph 50.55(e) of 10 CFR 50, this is submitted as an interim report.

During an outage of unit 1, MOV l-FCV-74-58 failed to operate properly. An investigation revealed the yoke broke from i the operator at the yoke-to-motor base welds. This was reported to W. S. Little, AEC-DRO, by telephone on May 14, 1974, as i a potential design deficiency with respect to Browns Ferry l units 2 and 3.

l Further investigation is under way to determine the exact cause of the apparent failure and corrective action required.

' We expect to furnish a final report of this deficiency by July 12, 1974.
