' Identification ~of Ocpurrence FCO 64-44 outboard refueling zone to SGTS crosstie damper failed to open during a routine surveillance test.
Conditions Prior to Occurrence The reactor vas in a cold shutdown condition.
Descrintion of Occurrence
' While conducting a routine surveillance test, the FC0 64-44 refueling zone to SGTS crosstie denper failed to open. The operator innediately opened the redundant refueling zone _.to SGTS crosstie darper FC0 64-45.-
Desirmation of Attarent Ccuse of Occurrence The air operated FC0 64-44 danper failed to open due to failure of FSV 64-h4 pilot 0010n id 7:17: t Operc.tc.
The defective valve was inspected, and dried grease was found on the core pole face which prevented the solenoid valve from operati: g when deenergized.
Analysis of Occurrence The FSV 64-44 pilot solenoid. valve stuck during the surveillance test and prevented the FC0 64-44 der:per from operating. The redundtnt FC0 64-45 dc::per was proven operable %ediately and would have operated satisfactorily had a primary contain-ment isolation signal occurred.
Corrective Action The FSV 64-44 pilot solenoid valve was replaced with an operable valve and tested satisfactorily.
Other ASCO valves of this type are being inspected for grease on the core pole face.-
