- Identification of Occurrence Motor failure of IFCI punp discharge valve FCV 73-44.
Conditions Prior to Occurrence The unit was in the cold chutdoun condition.
Descrintion of Occurrence During investigation of a ground on the 2507 de systen, HPCI punp discharge valve FCV 73-44 ras diccovered $noperable.
The notor had appexently been deraged during a previous cperation of the valve.
The motor was replaced and the valve returned to SerViCo.
Desi.~ nation of Aranrent Cauce of Occurrence The co: ratator van chorted and badb burned.
The bruch holders were burned and cne van grounded. The br:G:e shoes ucre charreu and blistered indicatin~ that the brake had enscz;cd with the zocor running.
The holding coil of the br de solenoid was open. Thic caused a continuous engaging rr.d dicengcG DG of the brake rhile i
the rotor ras running. This action caused the cnti-hipot capacitors to fail in the brde solenoid circuit and caused c :cessive chattering of the brushes on the conmutator uhich resulted in cevere arcing and finally motor failure.
Analysis of Occurrence Failure of the notor-oncreted pug discharge valve renders the' HPCI inoperable.
The unit was in the cold chutdoun condition at the tire of discovery and HPCI was not recuired. Failure during operation rould have prevented vessel injection.
All other redundcnt engineered safeguard cysters were available.
Corrective f.etion The notur was replaced and the valve tested and returned to service.
