4 ABNORFAL OCCURRDiCE REIORT Report No.:.BFAO-7436W Report Date: June 7, 1974 Occurrence Date: Fay 28, 1974 Facility: Browns Ferry nuclear Plant unit 1 Identification of Occurrence Spill of approxi. ate 4 12,000 gallons of potentially contaminated water to en uncontrolled area, unit 3 condensate backuach receiver tank room. Conditions Prior to Occurrence Valves 2-HCV-2-7h7 and 2-ECV-2-748 were removed end binnks were installed 'eefore startup of unit 1. This was part of interface between units 1 and 2 to prevent conteminated water from unit 1 getting to unit 2. Work Plan 1634 R1 had been issued to remove interface blanks and install above valves to connect units 1 end 2 condensate systems.
- Description of Occurrence t
When pipefitters loosened the flenge on the blenk for 2-HCV-2-7h7, unter flo::3d out in a gush. The pipefitters left the pipe tunnel without retightening the fic.nge bolts. This ellowed appro W ately 85,000 gallons of water to~be lost from unit 1 CST to pipe tunnel. The pipe tunnel overflowed to the units 1 end 2 condensate bachwach receiver tank room. Overflow frca the pipe tunnel also floved to unit 3 condensate ? backwash receiver tank room which is outside the controlled area. f Desimation of Arunrent Cruce of Occurrence h l The clearance for hold order 3h54 did not specify to close 1.-HCV-2-7h7. i k The flenge was not particily loos.ened as was reque::ted by the shift engineer. Irrediate Action a d The pipefitter general foreman clong with an assistent unit operator returned to 'j the tunnel to -(1) close valve 1-HCV-2-747 which had been inadvertently left open and (2) retighten the flange on blank 2-ECV-2-747. jn
- e Health physics employees and chemical laboratory employees were requested to start sampling the unit 3 sumps and the water in unit 3 condensate bachrach 1
receiver tank room. Inspection was made of pipe sleeves from pipe tunnel to unit 3 reactor builainc. Inspection showed no leaks of contaminated unter. [ Subscouent Action Samples were taken of all the sunpa in unit 3 reactor and turbine buildings es well as the water in unit 3 bachwash rocciver tank room. Isotopic analysis of above suples showed that ell ercas listed above were below the release limits on en ITC basis. Irregardless of the chove findings, water from unit 3 condensate backwash receiver pump roon which contained nost of the water 2nking to the unit 3 aren was punped with a te=porary pung to a floor drain in the unit 2 area. It then flowed to the floor drain collector tank in the redwaste building. Corrective Action A written procedure uns prepared to irclement renoving interface blanks and instelling valves 2-HC7-2-747 end 2-ECV-2-7kS. This procedure was followed and work was suceccafully completed on I'.ay 30, 1974 The Operations Supervisor held c discussion with e11 parties concerned on May 29, 1974. He stressed that greater etre cust be exercised in cicarance for work to be done. Discussion was also held with the Shift Engineer by the Plant Superintendent. e 6 k}}
