Conditions Prior to Occurrence .,

. Reactor was undergoing startup testing at approximately 1-percent power and 150 psig reactor pressure.

Description of Occurrence During RCIC testing at 150 psig, PdIG-71-1A vas observed indicating downscale.

Designation of Annarent cause of Occurrence An inspection of the instrument sensing lines on panel 25-7 revealed that the sensing lines were reversed on PdIS-71-1A.

satisfactorily and would have performed its intended function. These switches are part of the isolation logic for the RCIC stecs supply system.

Co'rrective Action -

The sensing lines were reversed on the local instrument panel and during a subsequent RCIC test, EdIS-71-1A'was confirnci to indicate upscale as required.

All instrument lines to differential pressure instruments listed in the technical specifications will be checked for correct installation either by physical inspection or by observation of instrunent response during initial unit.
