84 Response to Violation No. 413/84-19-01 1.
Duke Power Company admits the violation.

Part (1) of the violation is for a vertical run of 4"
- electray, located in the Auxiliary Feedwater Pump Room, in contact with the strut of mechanical hanger 1-R-CA-0232. Installation of this electray was complete on July 28, 1983 and received Electrical QC inspection on October 26, 1983. Erection of the mechanical hanger was complete on February 19, 1981 and received - Mechanical QC inspection on May 11, 1982.
" Support Inspection Instructions" M-51E, Serial Number CN-4, Paragraph D, requires Mechanical QC to verify freedom of move-ment on variable supports. Mechanical QC had performed the final walk of hanger 1-R-CA-0232 on November 10, 1983 and failed to iden-tify the problem. Therefore, the reason for this violation was the -
failure of the craft to follow design requirements for freedom of movement of supports and the failure of the inspector to detect this interference during final walk through inspection.
Part (2) of the violation pertains to 3/4" sleeve concrete anchors on pipe support 1-R-CA-0091 and 1/4" concrete anchors supporting lighting conduit, which violate the spacing requirements of Construc-tion Procedure CP-ll5. This problem is due to the installation of-field run electrical conduit, in the vicinity of QA Condition items (i.e., pipe supports), af ter the erection / inspection of QA Condition concrete anchors. The Craft personnel installing the Non-QA anchors violated CP-ll5 installation requirements. Preliminary investigation into the extent of this problem indicates it is -limited to - concrete anchors that are used to install field routed items (i.e., electrical conduit and instrument lines).

NCI 18743 was originated to-address the electray in contact with mechanical hanger 1-R-CA-0232. Electrical Craft shall rework the electray to obtain clearance between hangers.
NCI 18100 was originated to address the anchor spacing problem.
Design has resolved this situation as acceptable.
4.
Technical Support and QA will review inspection requirements to establish a method to insure that any similar problems of restriction of movement on variable supports' are corrected and to prevent further violations.
'A statistical analysis will be performed to evaluate. concrete anchor spacing problems.
5.
Full compliance of this item will be achieved by May~1, 1984.
