Perry Nuclear Power Plant Docket Nos. 50-440; 50-441 Actuator Mounting Brackets f or Motor Operated Exhaust Louvers [RDC 93(84)]
Dear Mr. Keppler:
This letter serves as our final report pursuant to 10CFR50.55(e) concerning cracks found in welds on actuator brackets for motor operated exhaust louvers on the Diesel Generator Rooms' Ventilation System and the Emergency Service Water Pump House Ventilation System.
Mr. J. Konklin was first notified on February 3,1984, by Mr. E. Riley of The Cleveland Electric Illuminar.ing Company that this problem was being evaluated.
The report includes a description of the deficiency and results of our evaluation.
It has been determined that this condition is not reportable pursuant to 10CFR50.55 (e).
Description of Deficiency Cracks were found in welds on actuator mounting brackets for all 24 motor operated exhaust louvers on the Units 1 and 2 Diesel Generator Rooms' Ventilation Systems (M43) and all 4 motor operated exhaust louvers on the Units 1 and 2 Emergency Service Water Pump House Ventilation System (M32).
In addition, weld cracks were found in linkage tubes on 5 of the 28 louvers.
These actuators and louvers were supplied to Robert Irsay Company, our HVAC installation contractor under Specification 49-4549-00, by their subcontractor MCC Powers who in turn purchased them from Construction Specialties, Inc.
The Construction Specialties QA Program did not address welding.
Results of Evaluation Project Organization Engineering has reviewed the types of cracks found on actuator mounting brackets and on linkage tubes and concluded that the louvers associated with the brackets and linkage tubes would not have failed during or after a celsmic event.

... Mr. 'Jzmes G. K:pplsr April 27, 1984 Project Organization Quality Assurance has evaluated, via Construction Quality Section Audit 969, the Construction Specialties, Inc. QA Program and found that an adequate and effective program was in place.
Their QA Manual was not revised when the louver design was changed from bolted to welded connections.
The manual has now been revised.
Qualified welders and procedures were used during original fabrication and have now been requalified as part of this evaluation. Procurement, Material Certification, and receiving inspection documentation for the louver assemblies including weld material was submitted and reviewed during the audit. No problems were noted.
Nonconformance Reports MOCF-079 and MCCF-082 were issued to document the hardware deficiencies and track the repairs.
