Shoreham Nuclear Plant Cracked, Broken Engine Driven Lube Oil Pump Bosses Part 21 Report Gentlemen:
During factory tests of the first LILC0 Colt-Pielstick PC-2.3 Diesel Genera-1 tor Set, a broken boss on the engine driven lube oil pump discharge nozzle was discove;cd. The broken boss significantly reduced the sealing area between the pump and an adapter which screws into the boss. A second lube oil pump was examined and the boss area was found to be cracked.

Our Engineering Department has concluded that the cracking or breaking was I
the result of overtorquing of the tapered thread adapter which screws into the boss. Such cracking or breaking, if severe enough, could permit oil leakage sufficient to prevent operation of an engine.
To prevent a reoccurrence, the adapter is being changed to straight threads with an undercut. The threaded hole in the boss has straight threads and need not be modified. All PC-2, PC-2.3 and PC-2.5 Diesel Engines shipped to the attached list of plants will be inspected for cracks or breakage and have their adapters changed to the straight thread design.
We will keep you advised of our progress in the field inspections and change-outs.
A copy of this report is being sent to the utilities involved.
