Reply'to a Notice of Violation San Onofre Nuclear Generating Station, Units 2 and 3

References:

1) Letter, Mr. Samuel J. Collins (USNRC) to Mr. Harold B. Ray (Edison), dated April 12, 1995
2) Letter, Mr. R. W. Krieger (Edison) to USNRC Document Control Desk, dated March 1, 1995 Reference 1 provided the results of a routine security inspection by Mr. D. Schaefer for the period March 20-24, 1995.

The letter also included a Notice of Violation (361/95-05-02) for granting initial unescorted access into the Protected Area (PA) prior to receipt of'the results of the fitness-for-duty drug and. alcohol tests.

Reference 2 provided the required 30-day written Licensee Event Report (LER) for the above noted occurrences involving incomplete PA access authorization processing.

These occurred during the Unit 2 pre-outage period when approximately 778 authorizations were being processed.

The reason for the violation was inattention to detail by the clerk processing the access authorization documents, and a failure to exercise due diligence in supervisory oversight by the supervisor.

As noted in Reference 2, Edison has replaced the clerk involved and counselled the access authorization staff.

In addition, Edison made a computer modification in the Training Records Information Management System (TRIMS) to minimize the potential for a security badge being issued before satisfactory completion of'the drug screening process.

Edison has also issued a desk guide for security badge issuance.
